Inflammatory Bowel Disease

Definition (NEJM 2002:347:417)

• Ulcerative colitis (UC): idiopathic inflammation of the colonic mucoso

• Crohn's disease (CD): idiopathic transmural inflammation of the Gl tract, skip areas

• In 5-10% of Pts with chronic colitis a clear distinction between UC and CD cannot be made even with mucosal biopsy ("indeterminate colitis") Differential diagnosis

• Infectious: bacterial (SSCY, £. co/i 0157:H7). pseudomembranous, amebic, CMV, STDs

• Ischemic colitis

• Intestinal lymphoma or carcinoma: collagenous colitis

• Irritable bowel syndrome

• Drugs (NSAIDs, OCR gold, allopurinol, penicillamine)
