Defects of the concha and preauricular region may sometimes require a flap that does not follow the RSTLs, especially in the case of elderly patients where swift treatment and rehabilitation are a prior ity. We have repaired these cases with an inferiorly based (Fig. 8.22) or temporally based transposition flap (Fig. 8.23 1), using an anteriorly based cheek rotation to close the secondary defect (Figs. 8.22 and 8.23, 2).
