The mucosal epithelium of the alimentary tract represents a crucial barrier to a broad spectrum of noxious and immunogenic substances within the intestinal lumen. Impairment of the integrity of the mucosal epithelial barrier is observed in the course of various intestinal disorders, including inflammatory bowel diseases, celiac disease, intestinal infections, and various other diseases. Furthermore, even under physiological conditions proteases, residential flora, dietary compounds, or other factors may cause temporary damage to the epithelial surface. Healing of the intestinal epithelium is regulated by a complex network of highly divergent factors, among them a broad spectrum of structurally distinct regulatory peptides that have been identified within the mucosa. Parenteral iron therapy

Disease, such as inflammatory bowel disease, is present. It is also occasionally necessary in gluten-induced enteropathy and when it is essential to replete body stores rapidly (e.g. where severe iron deficiency anaemia is first diagnosed in late pregnancy), or when oral iron cannot keep pace with continuing haemorrhage (e.g. in patients with hereditary haemorrhagic telangiectasia). Patients with chronic renal failure who are being treated with recombinant erythropoietin are also likely to require parenteral iron therapy. In this situation, the demand for iron by the expanded erythron may outstrip the ability to mobilize iron from stores, leading to a 'functional' iron deficiency. Increased red cell loss at dialysis contributes to iron needs and oral iron therapy is usually inadequate to prevent an impaired response to erythropoietin. The use of ' hypochromic cells' for detection of functional iron deficiency is discussed on p. 36.

Seed Proteins And Functionality

Although some seeds are consumed by humans with little or no processing (e.g., boiled rice), the vast majority are processed by the food industry into a wide range of products. The ability to process seeds is determined by their functional properties, which in turn depend, to a large extent, on the seed proteins. Studies on weak and strong wheat flour dough showed that stress relaxation tests at high strain values could differentiate dough from high-protein and low-protein wheat cultivars (Safari-Ardi & Phan-Thien, 1998). Stress relaxation measurements on wheat flour dough and gluten in shear showed that relaxation behavior of dough could be explained by two relaxation processes a rapid relaxation over b.1 to 1b s and a slower relaxation occurring over 1b to 1b,bbb s (Bohlin & Carlson, 198b). The rapid relaxation process is related to small polymers that relax rapidly and longer relaxation time is associated with high molecular weight polymers found within gluten. Similarly, stress relaxation behavior of wheat dough, gluten, and gluten protein fraction obtained from biscuit flour showed two relaxation processes a major peak at short times and a second peak at times longer than 1b s (Li et al., 2bb3). Treatment of folate deficiency

Long-term folic acid therapy is required when the underlying cause of the deficiency cannot be corrected and the deficiency is likely to recur, for instance in chronic haemolytic anaemias such as thalassaemia major and sickle cell anaemia, and in chronic myelofibrosis. It may also be necessary in gluten-induced enteropathy if this does not respond to a gluten-free diet. Where mild but chronic folate deficiency occurs, it is preferable to encourage any improvement in the diet after correcting the deficiency with a short course of folic acid. In any patient receiving long-term folic acid therapy, it is important to measure the serum cobal-amin level at regular (e.g. once yearly) intervals to exclude the coincidental development of cobalamin deficiency.

Other tissues

Folate deficiency causes reduced regeneration of cirrhotic liver. Patients with gluten-induced enteropathy and those with sickle cell anaemia have also been reported to show stunted growth, which has been improved coincidentally with commencement of folic acid therapy, but it is not certain how much the growth improvement in these children was due to folic acid and how much to other, simultaneously administered vitamins. In the fragile X syndrome, sister chromatid exchange and DNA breaks are increased in vitro in a folate-deficient medium, apparently at the Xq28 site. No in vivo abnormality of folate metabolism can be detected.


Malabsorption of dietary folate occurs in tropical sprue, in gluten-induced enteropathy in children and in adults, when it is associated with dermatitis herpetiformis. In the rare congenital syndrome of selective malabsorption of folate, there is an associated defect of folate transport into the cerebrospinal fluid, and these patients show megaloblastic anaemia, responding to physiological doses of folic acid given parenterally but not orally. These patients also show mental retardation, convulsions and other central nervous system abnormalities. Minor degrees of malabsorption may also occur following jejunal resection or partial gastrectomy, in Crohn's disease and in systemic infections but, in these conditions, if severe deficiency occurs, it is usually largely due to poor nutrition.

Extensional Flow

There are many test methods that measure the uniaxial extensional properties of dough such as the Brabender Extensograph and the Stable Microsystems Kieffer dough and gluten extensibility rig (Dobraszczyk & Morgenstern, 2003). However, these methods do not give rheological data in units of stress and strain since sample geometry is not defined or measured, and dimensions change nonuni-formly during testing. Therefore, it is impossible to determine stress, strain, modulus, or viscosity by these methods.

As poly(epsilon-caprolactone), and from diverse matrix proteins, such as collagen, elastin, fibrinogen, and silk fibroin (Boland et al., 2004 M. Li et al., 2005 W. Li et al., 2003 Matthews et al., 2002 McManus et al., 2006 Pham et al., 2006 Shields et al., 2004). Electrospun protein materials have fiber diameters in the range of those found in native ECM and display improved mechanical properties over hydrogels. The electrospun scaffolds may incorporate additional important ECM components, such as particular subtypes of collagen, glycosaminoglycans, and laminin, either in the spun fibers or as coatings, to promote cell adhesion, growth, and differentiation (Ma et al., 2005 Rho et al., 2006 Zhong et al., 2005). The use of specialized proteins such as silk fibroin offers the opportunities to design scaffolds with enhanced strength or other favorable features (Ayutsede et al., 2006 Jin et al., 2004 Kim et al., 2005 Min et al., 2004), while the use of inexpensive materials such as wheat...


In some disorders such as sickle cell disease, gluten-induced enteropathy (coeliac syndrome) and dermatitis herpetiformis, hyposplenism occurs frequently it is seen less frequently in Crohn's disease, ulcerative colitis and essential thrombocy-thaemia, and it occurs only occasionally in the other conditions listed in Table 21.2. Congenital absence of the spleen is rare and may be associated with organ transposition and with severe malformations of the heart and lungs.

Lead poisoning

Sideroblastic anaemia associated with pyridoxine deficiency has been described, although not completely documented, in a few patients with gluten-induced enteropathy, in pregnancy, and with haemolytic anaemias, such as sickle cell disease and mechanical or autoimmune haemolytic anaemia. Sideroblastic anaemia may be found as a complication of antituberculous chemotherapy, particularly with isoniazid and cycloserine (pyridoxine antagonists). Sideroblastic anaemia occurs in alcoholism if there is associated malnutrition and folate deficiency. Suggested mechanisms include interference with haem formation and pyridoxine metabolism. The anaemia rapidly reverses with abstinence from alcohol, a normal diet and pyridoxine therapy. Chloramphenicol inhibits mitochondrial protein synthesis and in some patients causes ring sideroblast formation, Pyridoxine therapy is almost always ineffective in primary acquired sideroblastic anaemias. In the present chapter I have focused on four aspects of seed composition that determine the quality for major end uses the nutritional quality of cereals (lysine content) and legumes (methionine content) and the processing properties of wheat gluten proteins and soybean glycinin. These examples illustrate the range of approaches that can be taken to improve quality, including the expression of genes encoding novel proteins from other plant species, the up-regulation (by inserting additional gene copies) or down-regulation (by antisense or cosuppression) of endogenous genes, and the construction and expression of mutant or ab initio designed genes. It is not surprising that amino acid composition has been one of the early successes as this can be readily defined at the molecular level.
