Diseases Caused By Fungi Dermatophyte Infections and Malasseziosis Aetiology and Pathogenesis

Superficial fungal infections by dermatophytes are cosmopolitan and affect any anatomical site, including scalp and nails however, one of the commonest presentations in the traveller affects one or both feet. These fungi are transmitted to humans by direct skin contact from their habitat in the soil, vegetation, or other individuals. Local conditions on the skin, such as a moist and hot environment while travelling, are predisposing factors. Derma-tophyte infections are highly prevalent in tropical climates as this represents an ideal environment for these organisms numerous case series and epidemiological studies from Latin America have been reported to the Spanish and Portuguese literature. The main genera involved in human infections are Trychophyton, Epidermophyton, and Microsporum, but infections of the foot including the toenails, are particularly caused by T. rubrum, T. mentagrophytes, and E. floccosum. Dermatophytes are kera-tinophylic organisms and exert their pathogenesis...

Individuals of both sexes and all age groups are affected by dermatophytes however, children under the age of 10 rarely present with tinea pedis. The main clinical pictures are those of localised tinea pedis, interdigital, plantar hyperkeratotic, and onychomycosis. Common names for these conditions include ringworm and athlete's foot. Dermatophyte infections can manifest as localised single or multiple circinate plaques with erythema and variable degrees of scaling on the body in cases of tinea corporis (Figure 9.17). Well Differentiated Thyroid Carcinoma

One cause of TC is the external beam radiotherapy widely used from 1920 to 1960 in children for treating benign conditions of the head, neck, and upper chest, such as tinea capitis, acne, tonsillar hypertrophy, and thymic enlargement. In 1949, Quimby and Werner 5 suggested a relationship between neck irradiation and the development of TC later in life. The thyroid gland in childhood is highly sensitive to radiation. After 1950,the use of radiotherapy for benign conditions gradually diminished. Currently, certain diagnostic techniques, especially fluoroscopy, can also involve radiation exposure in the range implicated in the pathogenesis of thyroid tumors 6 . Moreover, medically indicated radiation therapy for the treatment of assorted malignancies, especially acute lymphocytic leukemia and Hodgkin's disease, continues to be administered. Scattered radiation from the primary fields of therapy, which transverses thyroid tissue, results in sublethal thyroid irradiation in such patients.

Lymphoedema

M Conservative Skin or foot care, limb elevation, exercise, graduated compression stockings, massage, pneumatic massage devices are available. BP measurement or venesection should not be carried out from affected limbs. Medical Treatment of infection (e.g. antibiotics for cellulitis, antifungals for tinea pedis). Diuretics have no beneficial effects.

Fungal infections

The various clinical manifestations include favus, ringworm of the scalp, body, feet (athlete's foot) and nails some produce dyspigmentation, for example tinea versicolor. The lesions are mostly disfiguring but apart from the aesthetic aspect some are disabling, for example athlete's foot could lead to splitting of the skin and secondary bacterial infection. These infections have a worldwide distribution but some of the clinical manifestations such as ringworm of the feet tend to be more severe in the moist tropics. There are geographical variations in the incidence of various species, for example the predominant cause of tinea capitis in North America is Microsporon audouinii but it is Trichophyton tonsurans in South America. All age and racial groups are susceptible to these infections but some of the infections show variation with age and sex, for example ringworm of the scalp due to Microsporon audouinii is most prevalent in prepubertal children. Subclinical infections occur some...

Etiology

31 summarized seven studies on thyroid cancer incidence after the exposure of children to external radiation administered as therapy, often at low doses, for benign conditions such as tinea capitis. They found that the diagnosis of thyroid carcinoma was frequently not made until 25-35 years after exposure.
