Pathogenesis And Clinical Significance

Almost all of the organs of the human body can be infected by one or more of the spectrum of 14 microsporidian species described in the previous section. Many tissues and cell types are involved (Table 8.1). Cardiac disease and probable pancreatic, parathyroid and thyroid dysfunction have been reported for T. anthropophthera (Yachnis et al., 1996). Without treatment, the outcome is likely to be fatal for severely immunocompromised hosts infected with the disseminating species.

Both the injury itself and the surgery for the repair of these complex fractures may be associated with supraorbital nerve injury, loss of other ocular motor innervation due to damage near the superior orbital fissure, orbital emphysema and pneumocephalus, a subperiosteal haematoma with a secondary compressive optic neuropathy (Figure 14.7) and associated intracranial injuries. Late complications include persistent ptosis, due either to mechanical damage or denervation, lagophthalmos due to scarring and retraction of the upper eyelid or levator muscle and chronic or recurrent sinusitis, particularly that of the frontal sinus.

General Measures

All patients are considered to be at risk for aspiration of gastric contents due to the presence of ascites, upper gastrointestinal (GI) bleeding, encephalopathy, and the nonelective scheduling. Most patients are not premedicated with sedatives. A rapid sequence induction with cricoid pressure is used. Unless contraindi-cated, succinylcholine is used, since a prolonged block is not a concern. If the patient is a difficult intubation, an awake oral intubation is performed. Nasal intubation is avoided because of the potential for severe epistaxis and the concern of sinus infection in an immunocompromised patient who may have a prolonged intubation. Isoflurane in air plus narcotic is the usual anesthetic technique, as this minimizes depression of splanchnic blood flow. Long-acting drugs such as pan-curonium, lorazepam, and methadone may be used. Pulmonary Disease

Active cabin pressurization can adversely affect sinusitis and otitis. Air travel is contraindicated in those with pneumothorax or pneumomediastinum, and should be delayed for several weeks following middle-ear or thoracic surgery (Gong, 1991). The reduction in atmospheric oxygen during flight, while well tolerated by healthy travelers, can lead to marked hypoxemia in travelers with cardiopulmonary compromise, although the clinical significance of this is not completely clear (Gong, 1992). Dillard et al. (1991) found that pulmonary symptoms, including dyspnea, edema, wheezing, chest pain, and cyanosis, worsened during flight in 8 of 44 travelers (18.2 ) with chronic obstructive lung disease. Assessment of the need for supplemental oxygen is therefore recommended in travelers with underlying cardiopulmonary disease.
